New Patients

Registration with the practice

We would be happy to register any patients who live within the practice boundaries.

Patients from outside area can request to register depending on availability, but will need to make their own arrangements as no home visits or emergency call outs will be available.

There are 2 ways to register with our Practice. Either online or in person (paper forms):

  • To register online, please visit Register with a GP surgery (gp-registration.nhs.uk) and complete the online registration form. This will be sent through to us to register you on our system so you will be informed when you are registered with our Practice.
  • To register in person at the practice, you will need to complete a New Patient Registration Form (GMS 1), Practice Health questionnaire and Summary Care Records Form which are available at the reception.

Please bring with you the completed forms, 1 proof of address and 1 proof of photo ID with you when you come to register. For children 5 years of age and under the registration form only needs to be completed.

Then bring your forms in person to the reception, in the surgery opening times. Reception staff will check if all documents are completed correctly, check your proof of address and ID and book a New Patient Medical with one of our Nurses.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to contact the surgery and complete and return this form

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.
